Of course, there is always the possibility that someone incorrectly entered a duplicate External System Mapping, there is a greater chance that someone performed a merge between 2 client accounts, with each having an External System Mapping.
When you merge 2 accounts which each have an External System Mapping, both External System Mappings are captured for the merged account. This would cause duplicate External System Mappings on the new updated account:
