The CCO (Care Coordination Organization) name comes from the header that was configured for the staff action plan, for the specific program.
Example:
That reserved word ([CarePlanCCOName]) pulls the organization name from the Life Plan when you select 'external organization' as the author.
You can remove it from the header if there is no external organization by going into the Daily Activity & Work Queue Options settings in the configuration settings for the program and navigating to the Plan Printout Custom Header settings, in the Staff Action Plan Writing panel:
