How come I can't map fields to forms

Question:

I am trying to map the participant's last name, first name and SS# to some forms, but under the Data Mapping section, there is no Entity specified:

Why is this?

Answer:

The reason why you’re not able to map is because the form’s Primary Entity is not selected under Optional Data Mapping.

You have to go into each form and to its settings:

Then, you have to select Account Program for the Primary Entity:

After doing this, you should be able to map the forms.
