DDP Creation:
A DDP-1 is created when an individual starts a program.
A DDP-2 is created when the DDP form is complete, typically 30 days after starting the program, and will have an expiration date to ensure we can filter and have them all up to date.
Entering of Start/End Dates:
The start date for DDP-1s is when the client is placed in the program (i.e., when you put them in), and the end date is when they exit/leave the program (i.e., when you take them out).
DDP-2s have a start date and an end date, as they are done annually for residential and bi-annually in Day Services.
Expiration:
DDP-1s does not expire; they only end when someone is no longer in the program.
DDP-2s do expire and are either created annually or bi-annually.
