Answer

Concerns, complaints, and incidents all have many common attributes and can to varying degrees, be converted into each other

Fundamentally, most customers consider an "incident" to be something that implicitly demands regulatory intervention. Most customers need a way for their employees to report what they believe to be an incident (or any situation needing attention from the chain of command) and do not want to call those incidents, because those "concerns" may or may not be considered an incident from their regulating body. Many customers are required to accept anonymous input from 1st and 3rd parties (that may elevate to an incident) but are more frequently simple "complaints" (we hear example about kids running around, noisy music, landscaping, parking).
