Staff Action Plans (SAPs) are linked to Care Plans and Life Plans.
To build a new Care Plan...
Care Plans and Life Plans can be created in the Care Coordination section of the Engagement Menu in IMPOWR (click Add New Plan, and then select the individual the plan is being created for). Various dates (effective date, issue date, review date, etc.) can be added, as can the plan owner, author, plan type, and status. Each plan also has an option of adding addendums, approvals, and staff action plans.
The Care Plan types are configurable in the Configuration Reference Libraries under Plan Types). Within each plan type, you can check the box of whether they use the SAP or not. And if it does use that, you have the ability to create those. You can also identify if the Care Plan is generated from templates, whether it has an auto-expiration date, and who are the default approvers you want for that specific type of plan.
Under Care Plans, upload a PDF of the plan, in the Reference Document panel. You can also select the Staff Action Plan (SAP) tab to add/build a new plan or view and edit an existing SAP.
To build a new Staff Action Plan...
Step 1: Select a Program and the Plan Owner.
Step 2: Identify a POMS goal and click on the '+' button - pick one or more POMS goals from the chooser that opens (will be multi-select). The POMS goals are configurable in the Client Goal Library, found in the Client Care & Outcomes section of the Configuration Reference Libraries.
Step 3: Enter the Client Goal associated with that POMS goal (the goals are also configurable in the Client Goal Library).
Step 4: Enter the Outcome/Achievement Assessment that you are going to use to measure the outcome of that goal. (The assessments are user configurable in the Screening & Assesment Tools list in the Configuration Reference Libraries.)
Step 5: Enter the Provider Assigned Goal. (The Provider Assigned Goals are user configurable in the Care & Staff Action Plans panel in the Configuration Reference Libraries.)
Step 6: After selecting the goal, add the appropriate staff supports and enter the duration, the frequency, enter the months, and enter the location. We also have the ability to tie these to Z codes so when we do the managed care billings, it is already built in.
Printing of Plans
Staff Action Plans can be printed as well as printed in OPWDD format.
Approvals
They can be approved by clicking on the "Draft" status button. Once approved, the SAP will be locked from further changes. To unlock the plan, just click on the "Approved" status button.
Typically, Site Supversiors and Planning Coordinators create and submit the Staff Action Plans for approval, regardless of the department. The Manager of Services (MOS) in all departments approve them. The MOS is able to edit the plan, but the Service Administrators (the Directors) are the ones who would need to approve them and lock them. They should also be able to unlock them.
Only the person who locked it or their Supervisor should be the ones to unlock the plan. That would mean once unlocked, it would have to be approved again, which could change the dates and mess things up... so, if at all possible, staff really shouldn't be unlocking it once it is locked.
Copying
Plans can be copied from prior plans - just select which POMS to copy. POMS can also be copied.
Expiration Dates
The SAP expiration date default is typically 6 months, but this is user configurable based on type of plan. Life Plans do not expire until next Life Plan is in place. For SAPs, there will be a start/end date.
